What it is used for
Lowers LDL cholesterol to reduce the risk of heart attack and stroke.
How it works
A strong statin that lowers LDL cholesterol and heart risk.
How to take it
Usually 5 to 20 mg once a day, any time. People of Asian background often start on 5 mg.
Common side effects
- Muscle aches
- Headache
- Constipation
- Nausea
- Slightly higher sugar
Get medical help straight away if
- Unexplained muscle pain, weakness or dark urine (rare muscle breakdown)
- Yellow skin
Be careful
Report unexplained muscle pain.
Other medicines and interactions
Clarithromycin, itraconazole, some HIV medicines, gemfibrozil and large amounts of grapefruit juice raise statin levels and muscle risk. Can increase the effect of warfarin. Antacids reduce absorption; take 2 hours apart.
Pregnancy and breastfeeding
Stop statins if you become pregnant or are trying to conceive, and do not take while breastfeeding.
Clopidogrel
What it is used for
Prevents clots after a heart attack, stent or stroke.
How it works
An antiplatelet that stops platelets clumping, preventing clots after heart attacks, stents and strokes.
How to take it
Usually 75 mg once a day, with or without food. Do not stop without your heart doctor's advice, especially after a stent. Tell your dentist and surgeon.
Common side effects
- Bruising
- Nosebleeds
- Stomach upset
Get medical help straight away if
- Black stools, blood in urine or vomit
- Bleeding that will not stop
- Sudden severe headache
Be careful
Increases bleeding risk; tell your dentist or surgeon. Do not stop without your doctor.
Other medicines and interactions
Omeprazole and esomeprazole reduce its effect (pantoprazole is usually preferred). Bleeding risk higher with aspirin, NSAIDs, warfarin and SSRIs.
Pregnancy and breastfeeding
Used in pregnancy only if clearly needed; not recommended while breastfeeding.
Aspirin
What it is used for
In higher doses it relieves pain and fever; in low doses (75 to 150 mg) it helps prevent heart attacks and strokes.
How it works
At pain-relief doses it blocks prostaglandins; at low doses it permanently stops platelets from sticking together, which is why 75 to 150 mg a day helps prevent clots in people with heart disease.
How to take it
Low-dose aspirin is taken once a day after food, exactly as your doctor prescribed; do not stop suddenly without advice. For pain, adults take 300 to 600 mg every 4 to 6 hours after food, up to 4 g a day.
Common side effects
- Indigestion
- Easy bruising
- Nosebleeds
- Mild stomach bleeding over time
Get medical help straight away if
- Black or bloody stools or vomiting blood
- Wheezing
- Ringing in the ears (sign of too much)
- Unusual bleeding that does not stop
Be careful
Can cause stomach bleeding. Not for children or teenagers with fever (risk of Reye's syndrome).
Other medicines and interactions
Raises bleeding risk with warfarin, clopidogrel, ticagrelor, NSAIDs and SSRIs. Ibuprofen taken just before low-dose aspirin can block its heart-protecting effect. Raises methotrexate levels.
Pregnancy and breastfeeding
Low-dose aspirin is sometimes prescribed in pregnancy (for example to prevent pre-eclampsia); otherwise avoid pain-relief doses, especially in the last three months. Avoid regular use while breastfeeding unless a doctor prescribes it.
